Measure for the piece you are buying

Tops and layers: measure around the fullest part of your bust. Check shoulder, sleeve and body length separately when those affect your fit.
Bottoms: measure around your natural waist and the fullest part of your hips. Compare garment waist, rise and inseam with a pair you already like.
Keep the tape level and comfortably close to your body without pulling it tight. Body measurements and flat garment measurements are different.

Between sizes or choosing a combined size?

Use the measurement that matters most for the garment, then consider the cut and how you want it to fit. A combined label such as S/M, M/L or 1XL/2XL is one garment cut to cover both rows: compare your measurements with both rows and read the fit notes for that style. Do not automatically go up or down across every style.
